It is administered through the Oklahoma Department of Human Services (OKDHS) Aging Services and the Long-Term Care Authorities of Tulsa and Enid. Services can include adult day care, home delivered meals, comprehensive home care, case management, skilled nursing care, medications, specialized equipment and supplies, environmental modifications, and other supportive assistance. Eligibility requirements include an assessment by OKDHS nurses to determine level of care. For application & information call 1-800-435-4711 or at: www.okdhs.org/programsandservices/aging/adw/ Child Care Assistance This is a program of the OKDHS that helps to pay for all or part of the cost of child care for children up to age 13 while the parent(s) work or go to school. Assistance may also be available for children older than 13 who have a disability. Child care must be provided by a licensed provider. Eligibility for the program is based on income. Application for the program is made through the local county OKDHS Family Service Center. Applicants need to provide information about work or school schedule in addition to proof of income. You can find information and the application at: www.okdhs.org/programsandservices/cc/asst/ Developmental Disability Support Services Family Support Assistance Program This program provides cash payments ranging from $250 to $400 per month, depending on the number of children under 18 years old with developmental disabilities living in the home. To qualify, the family must be headed by natural or adoptive parent(s) or a legal guardian (adoptive parents cannot also receive an adoption subsidy); have an adjusted gross income of no more than $45,000 per year and live in Oklahoma. The child must be under 18 years old, a diagnosis of a developmental disability as defined by Oklahoma law; and live in the family home. Families can not receive the Family Support Assistance payment if they are already receiving waiver services. Application for this program are available at the local county OKDHS office or one of the Developmental Disability Services Division (DDSD) area offices and online at: www.okdhs.org/programsandservices/dd/commsvcs/docs/areacontactinfo.htm Families who qualify are served in chronological order, based on the date of their application, as funds allow. Home and Community Based Waiver This program offers a variety of services so that individuals with a developmental disability can live at home. The waiver provides services such as in-home paraprofessional supports, respite care, specialized therapy, counseling, family training, adaptive equipment, architectural modifications to the home, skilled nursing, transportation, and habilitation training. To be eligible for this program, the person or child must: 1.) be a resident of the State of Oklahoma; 2.) be determined to have a disability, with a diagnosis of mental retardation, by the Social Security Administration or the Oklahoma Health Care Authority, Level of Care Evaluation Unit; 3.) be three years of age or older; 4.) be determined by the Oklahoma Health Care Authority, Level of Care Evaluation Unit, to meet the ICF/MR Institutional Level of Care requirements; 5.) be determined financially eligible through the DHS Family Support Services Division; 6.) not be enrolled in any other Medicaid waiver program; and 7.) not be residing in a hospital, nursing facility, or ICF/MR (Intermediate Care Facility for the Mentally Retarded). There is a waiting list for this program. Although children are eligible at age three (3), families are encouraged to apply as early as possible. Applications are available at local OKDHS Family Service Centers, area DDSD offices and online at: www.okdhs.org/programsandservices/dd/commsvcs/docs/areacontactinfo.htm In Home Supports Waiver In this program, families and individuals can choose the service(s) they need. Those eligible may receive services up to a specific amount each year. All services are provided through agencies which contract with DDSD. Services include respite, habilitation training, environmental accessibility adaptations, family training, adaptive equipment, and specialized medical supplies. Other services are available to children through the Medicaid EPSDT program and the SSI-Disabled Children’s Program. Eligibility includes meeting requirements for the Home and Community Based Waiver. The individual must reside in the family's home or his or her own home, and have critical support needs that can be met through a combination of non-waiver and Medicaid State Plan resources available to the individual, and within the per capita waiver allowance. There is a waiting list for this program. Application is through the local county OKDHS Family Service Center or one of the area DDSD offices. Applications can also be found at: www.okdhs.org/programsandservices/dd/commsvcs/docs/areacontactinfo.htm Supplemental Nutrition Assistance (Food Stamps) The Supplemental Nutrition Assistance Program, formerly the Food Stamp program is a federally funded program that provides benefits to households who meet eligibility requirements established by the U.S. Department of Agriculture (USDA). The monthly benefit is determined by the income and size of the household and is used to purchase food. The food stamp benefit in most instances is intended to supplement other income. Anyone can apply for food stamps at the OKDHS Family Service Center in the county where he/she lives. Application can be found at: www.okdhs.org/library/forms/fssd/default.htm Low-Income Home Energy Assistance Program (LIHEAP) OKDHS, in cooperation with the federal government, provides seasonal assistance to low-income households to assist with winter heating bills, and to provide help for some families who have received utility cut-off notices. Applications are taken in the local OKDHS Family Service Center. LIHEAP program application periods are dependent on available funding. Heating assistance applications are usually taken for two weeks beginning the first Monday in December. Crisis assistance applications are usually taken for two weeks starting March 15th or the first Monday following March 15th. Summer Cooling assistance applications are usually taken for two weeks starting July 15th or the first Monday following July 15th. Medicaid This program is also known as Title XIX (19), or Health Benefits. Services range from preventive medical care for children and adults to nursing care and alternative homebound care. It is not necessary for a medical condition to exist before applying for services. Eligibility for Medicaid is determined by federal and state regulations. Applicants must meet financial and other eligibility conditions to qualify for services. Those who might be eligible include: someone who is aged (age 65 or older), blind (as determined by Social Security Administration), disabled (as determined by Social Security), pregnant, a child under the age of 19, and adults who have children under the age of 19 whose primary place of residence is with the adult. Information on eligibility requirements and an application can be found at www.okdhs.org/programsandservices/health/med/docs/elig.htm#apply You may also request an application by phone or mail. This form may be completed and sent to your local county office. Appointments may be scheduled for your convenience. TEFRA (Tax Equity and Fiscal Responsibility Act of 1982) gives states the option to make Medicaid benefits available to children with physical or mental disabilities who would not ordinarily be eligible for Supplemental Security Income (SSI) benefits because of their parent's income or resources. This option allows children who are eligible for institutional services to be cared for in their homes. This option went into effect in Oklahoma October 1, 2005. Application can be made at any OKDHS office. Eligibility information and applications are available at www.okdhs.org/programsandservices/health/tefra/ Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Persons under 21 who are receiving Medical Benefits / Medicaid may receive health and dental screenings and follow-up treatment for certain conditions. Providers include physicians, health departments, schools and some local health clinics. These exams are available on a periodic schedule based on the age of the individual. For more information: www.okdhs.org/programsandservices/health/epsdt/ State Supplemental Payment (SSP) This state funded program provides financial assistance to low income, aged, blind and disabled population who have been determined financially eligible. These individuals also receive Medicaid Services. Application is made through the local OKDHS Family Service Center. Supplemental Security Income (SSI) The SSI program is administered by the Social Security Administration and provides monthly income to people who are blind or disabled, or age 65 and older, and have limited income and financial resources. To qualify for SSI, a child must be blind or disabled and financially needy. In determining a child’s financial eligibility, the parents’ or step-parents’ income and resources are considered. Determining disability eligibility is done by the state’s Disability Determination Unit using SSA rules. Generally Social Security considers a child disabled if the child has “marked and severe functional limitations” from a physical or mental condition and if the condition has lasted or is expected to last at least 12 months or will result in the child’s death. Apply for SSI at local Social Security offices. Supplemental Security Income - Disabled Child Program (SSI-DCP) This program assists children from birth to age 18 who receive a Supplemental Security Income Disability Payment (SSI) to receive assistance in obtaining needed equipment and services. This includes adaptive equipment, specialty formula from birth to age 18, diapers from age 4 to age 18, and other services. Application should be made through the local county OKDHS Family Service Center. Temporary Assistance to Needy Families (TANF) TANF is a cash assistance program available for families who have children deprived of support because of a parent’s death, incapacity, absence, or unemployment. The purpose of this federal program is to provide temporary support in meeting basic needs, training leading to employment, employment services, and day care assistance for qualified families with children. While receiving cash assistance, the adult recipients are required to work or participate in activities geared toward work and achieving self-sufficiency. Recipients also receive Health Benefits (Medicaid). Persons receiving TANF also receive child support collection services from Child Support Enforcement. Everyone has the right to apply for TANF money payments and to be treated with courtesy and respect. Applications are made at the OKDHS Family Service Center n the county where the family lives. Women, Infants and Children (WIC) Nutrition Program WIC is a federal program administered by the Oklahoma State Department of Health. The program provides food, formula, nutritional screening, and information. Those eligible include women who are pregnant, breastfeeding or who have recently given birth, as well as infants and children to five years of age. Income guidelines apply. Application is made at a local WIC clinic or county health department. It is administered through the Oklahoma Department of Human Services (OKDHS) Aging Services and the Long-Term Care Authorities of Tulsa and Enid. Services can include adult day care, home delivered meals, comprehensive home care, case management, skilled nursing care, medications, specialized equipment and supplies, environmental modifications, and other supportive assistance. Eligibility requirements include an assessment by OKDHS nurses to determine level of care. For application & information call 1-800-435-4711 or at: www.okdhs.org/programsandservices/aging/adw/ Child Care Assistance This is a program of the OKDHS that helps to pay for all or part of the cost of child care for children up to age 13 while the parent(s) work or go to school. Assistance may also be available for children older than 13 who have a disability. Child care must be provided by a licensed provider. Eligibility for the program is based on income. Application for the program is made through the local county OKDHS Family Service Center. Applicants need to provide information about work or school schedule in addition to proof of income. You can find information and the application at: www.okdhs.org/programsandservices/cc/asst/ Developmental Disability Support Services Family Support Assistance Program This program provides cash payments ranging from $250 to $400 per month, depending on the number of children under 18 years old with developmental disabilities living in the home. To qualify, the family must be headed by natural or adoptive parent(s) or a legal guardian (adoptive parents cannot also receive an adoption subsidy); have an adjusted gross income of no more than $45,000 per year and live in Oklahoma. The child must be under 18 years old, a diagnosis of a developmental disability as defined by Oklahoma law; and live in the family home. Families can not receive the Family Support Assistance payment if they are already receiving waiver services. Home and Community Based Waiver This program offers a variety of services so that individuals with a developmental disability can live at home. The waiver provides services such as in-home paraprofessional supports, respite care, specialized therapy, counseling, family training, adaptive equipment, architectural modifications to the home, skilled nursing, transportation, and habilitation training. To be eligible for this program, the person or child must: 1.) be a resident of the State of Oklahoma; 2.) be determined to have a disability, with a diagnosis of mental retardation, by the Social Security Administration or the Oklahoma Health Care Authority, Level of Care Evaluation Unit; 3.) be three years of age or older; 4.) be determined by the Oklahoma Health Care Authority, Level of Care Evaluation Unit, to meet the ICF/MR Institutional Level of Care requirements; 5.) be determined financially eligible through the DHS Family Support Services Division; 6.) not be enrolled in any other Medicaid waiver program; and 7.) not be residing in a hospital, nursing facility, or ICF/MR (Intermediate Care Facility for the Mentally Retarded). There is a waiting list for this program. Although children are eligible at age three (3), families are encouraged to apply as early as possible. Those eligible may receive services up to a specific amount each year. All services are provided through agencies which contract with DDSD. Services include respite, habilitation training, environmental accessibility adaptations, family training, adaptive equipment, and specialized medical supplies. Other services are available to children through the Medicaid EPSDT program and the SSI-Disabled Children’s Program. Eligibility includes meeting requirements for the Home and Community Based Waiver. The individual must reside in the family's home or his or her own home, and have critical support needs that can be met through a combination of non-waiver and Medicaid State Plan resources available to the individual, and within the per capita waiver allowance. Applications can also be found at: www.okdhs.org/programsandservices/dd/commsvcs/docs/areacontactinfo.htm Supplemental Nutrition Assistance (Food Stamps) The Supplemental Nutrition Assistance Program, formerly the Food Stamp program is a federally funded program that provides benefits to households who meet eligibility requirements established by the U.S. Department of Agriculture (USDA). The monthly benefit is determined by the income and size of the household and is used to purchase food. The food stamp benefit in most instances is intended to supplement other income. Anyone can apply for food stamps at the OKDHS Family Service Center in the county where he/she lives. Application can be found at: www.okdhs.org/library/forms/fssd/default.htm Low-Income Home Energy Assistance Program (LIHEAP) OKDHS, in cooperation with the federal government, provides seasonal assistance to low-income households to assist with winter heating bills, and to provide help for some families who have received utility cut-off notices. Applications are taken in the local OKDHS Family Service Center. LIHEAP program application periods are dependent on available funding. Heating assistance applications are usually taken for two weeks beginning the first Monday in December. Crisis assistance applications are usually taken for two weeks starting March 15th or the first Monday following March 15th. Summer Cooling assistance applications are usually taken for two weeks starting July 15th or the first Monday following July 15th. Medicaid This program is also known as Title XIX (19), or Health Benefits. Services range from preventive medical care for children and adults to nursing care and alternative homebound care. It is not necessary for a medical condition to exist before applying for services. Eligibility for Medicaid is determined by federal and state regulations. Applicants must meet financial and other eligibility conditions to qualify for services. Those who might be eligible include: someone who is aged (age 65 or older), blind (as determined by Social Security Administration), disabled (as determined by Social Security), pregnant, a child under the age of 19, and adults who have children under the age of 19 whose primary place of residence is with the adult. This form may be completed and sent to your local county office. Appointments may be scheduled for your convenience. TEFRA (Tax Equity and Fiscal Responsibility Act of 1982) gives states the option to make Medicaid benefits available to children with physical or mental disabilities who would not ordinarily be eligible for Supplemental Security Income (SSI) benefits because of their parent's income or resources. This option allows children who are eligible for institutional services to be cared for in their homes. This option went into effect in Oklahoma October 1, 2005. Application can be made at any OKDHS office. Eligibility information and applications are available at www.okdhs.org/programsandservices/health/tefra/ Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Persons under 21 who are receiving Medical Benefits / Medicaid may receive health and dental screenings and follow-up treatment for certain conditions. Providers include physicians, health departments, schools and some local health clinics. These exams are available on a periodic schedule based on the age of the individual. For more information: www.okdhs.org/programsandservices/health/epsdt/ State Supplemental Payment (SSP) This state funded program provides financial assistance to low income, aged, blind and disabled population who have been determined financially eligible. These individuals also receive Medicaid Services. Application is made through the local OKDHS Family Service Center. Supplemental Security Income (SSI) The SSI program is administered by the Social Security Administration and provides monthly income to people who are blind or disabled, or age 65 and older, and have limited income and financial resources. To qualify for SSI, a child must be blind or disabled and financially needy. In determining a child’s financial eligibility, the parents’ or step-parents’ income and resources are considered. Determining disability eligibility is done by the state’s Disability Determination Unit using SSA rules. Generally Social Security considers a child disabled if the child has “marked and severe functional limitations” from a physical or mental condition and if the condition has lasted or is expected to last at least 12 months or will result in the child’s death. Apply for SSI at local Social Security offices. Supplemental Security Income - Disabled Child Program (SSI-DCP) This program assists children from birth to age 18 who receive a Supplemental Security Income Disability Payment (SSI) to receive assistance in obtaining needed equipment and services. This includes adaptive equipment, specialty formula from birth to age 18, diapers from age 4 to age 18, and other services. Application should be made through the local county OKDHS Family Service Center. Temporary Assistance to Needy Families (TANF) TANF is a cash assistance program available for families who have children deprived of support because of a parent’s death, incapacity, absence, or unemployment. The purpose of this federal program is to provide temporary support in meeting basic needs, training leading to employment, employment services, and day care assistance for qualified families with children. While receiving cash assistance, the adult recipients are required to work or participate in activities geared toward work and achieving self-sufficiency. Recipients also receive Health Benefits (Medicaid). Persons receiving TANF also receive child support collection services from Child Support Enforcement. Everyone has the right to apply for TANF money payments and to be treated with courtesy and respect. Applications are made at the OKDHS Family Service Center n the county where the family lives. Women, Infants and Children (WIC) Nutrition Program WIC is a federal program administered by the Oklahoma State Department of Health. The program provides food, formula, nutritional screening, and information. Those eligible include women who are pregnant, breastfeeding or who have recently given birth, as well as infants and children to five years of age. Income guidelines apply. Application is made at a local WIC clinic or county health department.
